Provide community support and case coordination services for a caseload of consumers with disabilities, serving persons with developmental disabilities, mental illness, and/or physical disabilities. Assess consumers overall functioning, identifying problems, needs, skill strengths and deficits, and assume a lead role in the development of the consumer centered plan. Implement selected aspects of the plan including skill building, counseling, advocacy, and building community, family, and natural supports. Ensure consumer linkages to community service providers, entitlement programs and housing alternatives.
Bachelor's degree (B.A./B.S) from four-year college or university in the human services field and must have at least one year of direct care experience working with persons with disabilities. Must be able to achieve Qualified Intellectual Disability Professional (QIDP) status to work with persons with developmental disabilities.
Must be able to communicate respectively and effectively in both oral and written form; must have good social and assessment skills; must have the ability to work independently and interact appropriately within the public domain.

$12.50 - $15.00 per hour
