Community Services AssistantMacomb, IL
Provide intensive community based services to a caseload of consumers with developmental disabilities or mental illness with appropriate and documented diagnosis. Participate in assessment of consumers' overall functioning and the development, review, modification, and implementation of Consumer Centered Plans (CCP). Provide skills training and rehabilitation and recovery oriented interventions to consumers and families, as applicable, within their home environment or community setting.
High School diploma/GED with 1 year related experience and/or training. Must be 21 years of age or older. Experience in human service field desirable. Must at a minimum, meet the DMH credentialing requirements of a Rehabilitative Services Associate (RSA) and achieve DSP status.
Must be able to communicate effectively both verbally and in writing, have good social and assessment skills, have the ability to work independently and interact professionally within the public domain, and have the commitment to work with the target population(s) served. Ability and willingness to work flexible hours, as needed. As applicable, must demonstrate belief in the concepts of empowerment and recovery for persons with serious mental illness.
Must have reliable transportation, valid driver license, proof of insurance and good driving record. Must obtain and maintain CPR and First Aid certification after hired; must attend Blood Borne Pathogen and OIG training after hired and annually thereafter. Must successfully complete DSP training and other trainings as assigned. If licensed, required to maintain license requirements.
