System Care Coordinator

Galesburg, IL

Duties

Primarily responsible for provision comprehensive and effective development and oversight to the Recovery Oriented System of Care within the four county area. Work directly with the Bridgeway Behavioral Health, Substance Use, Crisis/SASS, School Based Counseling, Community Housing/Support and Employment services and personnel an all locations. Develop appropriate operational policies, procedures, documentation, etc. for the System of Care in coordination with stakeholders. Provide innovative coordinated leadership with the Behavioral Health Services team and others to assure comprehensive communication, integration of treatment services and coordination with external resources. Educate and train Bridgeway staff as well as external organizations and stakeholders in areas related to the System of Care. Oversee the development and accomplishment of short and long term goals/strategies and be an active participant in the planning and evaluation system. Monitor the financial performance for related programs on a monthly basis as applicable.


Education

Master's degree from a four year college or university in Behavioral Health, Substance Use or related field preferred; or Bachelor's degree in Behavioral Health, Substance Use or related field. Experience in coordinating operational system preferred.


Skills

Must be able to communicate effectively both verbally and in writing, have good social, assessment and organization skills, and be able to work as a team member. Must have a commitment to work with persons with serious mental illness and demonstrate a belief in the principles of recovery and empowerment for persons with serious mental illness. Willingness to work flexible hours and basic computer navigation and word processing skills essential.

Salary Range

Master's$16.83 - $20.20/hr
Bachelor's$14.00 - $16.80/hr
